The hand-wash sink in the front was blocked by three large, glass ornaments and not accessible at all times for employee use. The ceiling in the back where the chest freezers, single-service items and reach-in cooler are kept had exposed raw wood and loose insulation. Side window in back of pizza prep unit in the back was open, not screened and did not protect against the entry of insects.
